Our staff maintains the confidentiality of the records of our clients. These records document lab reports, visits, and all other procedures the client had while at our facility.

Make A Request

Please call our front office at 541-426-4524 to start the process of getting your records. 

You may fill out and print request forms in advance, sign them by hand and submit the documents via fax (541) 426-3035 or in person at 606 Medical Parkway in Enterprise. Please call us to let us know you are faxing the forms to us.
